Case Manager - Target

Prospect, one of Alberta’s most innovative and leading non-profits, is seeking experienced Case Managers to join our supported employment team. You will have experience working with people with a broad range of disabilities including mental illness and developmental disabilities. Your approach to working with stakeholders and partners is highly innovative, creative, dynamic, collaborative, and outcomes focused.
Reporting to the Program Manager, you will provide direct services to clients with disabilities who are seeking support in acquiring meaningful employment opportunities. You will work with a highly motivated staff team who specialize in employment strategies and supports aimed at increasing workforce participation of people with disabilities.
Responsibilities include fostering and building relationships with a variety of stakeholders, client case management, developing and maintaining service plans, and maintaining sound knowledge of the referral and service provider community.
In addition, you will become the central point of contact for clients, support networks and community contacts. You will ensure the referral community is equipped with accurate and relevant program information through regular contact and community presentations.

QUALIFICATIONS:
• Sound knowledge of diversity, inclusion strategies, and people with disabilities
• Understanding of Employment and Career Service Agencies for persons with Disabilities
• Exceptional interpersonal effectiveness
• Excellent presentation, written and verbal communication skills
• Proven ability to manage details with a strong focus on quality and outcome achievement
• Superior customer relations skills
• Proficient in Microsoft Outlook, Excel, Power Point, contact management software
• Related post secondary degree
